At Care Pathway Navigators we can help you navigate the various aged care services available, and assist you in securing funding and finding the right care for you or your loved ones.
Do you have a disability and need support to help you live independently? Do you have a child with special needs and are struggling to get appropriate services in place to support you and your family? Do you have a support plan in place but your needs have changed and your current plan needs upgrading?
Care Pathway Navigators takes care of the process for you.
If you are DVA eligible and are finding you need assistance to remain independently living in your own home, Care Pathway Navigators can assist you to access support via the Department of Veterans Affairs.
